What care management covers

Care management is the work of a care partner: planning and reviewing the participant's services, coordinating providers and keeping in touch with the participant and their family.

How it's funded

  • 10% of the quarterly budget is set aside for care management.
  • Participants pay no contribution on care management.
  • It's claimed from its own account, separate from services.

Keep it separate

Track care management in its own account so spending on services never eats into it, and record each care partner's contacts on the participant's record.

How Care Nexus helps

Care Nexus keeps care management in its own account, so it's never spent on services.

  • Care Nexus creates the claims and invoices automatically from the services delivered, with care management claimed from its own account. Once you've reviewed them, they're sent to the right place automatically.
  • Each participant's care management funding is tracked separately from their services budget.
  • No contribution is charged on care management.
